Forms required to employ student workers-Main Campus
 
(Make sure all are signed and please assemble in the following order):

  1. Ohio Department of Taxation - Employee's Withholding Exemption Certificate
     
  2. W-4 form - Students to complete this online, under MyUT tab – You’ll be able to access this form, after you’ve been added to the system.
     
  3. Ohio Public Employee Retirement System (OPERS form): 
        
    a) Request for Optional Exemption As a Student; - use this form if you do not want to enroll in the Public Employees Retirement Plan.
     
    OR
    b) Enrollment - Personal History Record – use this form if you do want to enroll – 10% of your gross wages will be deducted; MUST ENROLL IF NOT TAKING CLASSES FOR THE SUMMER.
     
  4. Statement Concerning Your Employment in a Job Not Covered by Social Security (SSA-1945)  (Use only if enrolling in OPERS)
     
  5. Direct Deposit - Students to complete this online, under MyUT tab – You’ll be able to do this once you’re in the system.
     
  6. Personal Information Form  (with VA and War Statement attached)
     
  7. DMA Public Employment (with terrorist list attached)
     
  8. Employment Eligibility Verification Form (I-9)   
    IMPORTANT: Be sure that start date on the SEPA corresponds with the date on the I-9. 
    (Must have table of acceptable documents copied on back)
