Once you pick up your Work-Study Contract from Financial Aid, please be sure to complete the following documents and return them to the Office of Professional Development (Room 1030) as soon as possible. Please complete only the documents in the list below that apply to you and submit them for review in the order listed.
You are not able to begin working until your paperwork has been processed.

Once your paperwork is processed by Payroll and your “Employee” tab appears in the MyUT system, you must electronically complete your W-4 form and Direct Deposit. You will receive further instructions.
**Important Notes for Students Who HAVE PREVIOUSLY PARTICIPATED in Work-Study During Law School:

Employee’s Withholding Exemption Certificate (Ohio or Michigan)
* For Ohio, school district codes must be completed and can be found on tax.ohio.gov
Form I-9 Employment Eligibility Verification
* Student must bring original identification documents to the OPD so that the staff member signing the paperwork on behalf of the university can certify that the documents appear genuine. Please review the list of acceptable documents on the last page of the I-9 form. Student must bring:
One document from List A OR
One document from List B AND one document from List C
*Students who are NOT enrolled in classes for summer and are participating in Work-Study must enroll in the Ohio Public Employees Retirement System by completing this form. 10% of your gross wages will be deducted.
*Supervisor e-mail is especially important since the law school will use this for communications with supervisor.
Request for Optional Exemption as a Student (OPERS)
If you are currently registered for classes, you may complete this to request an exemption from wage withholdings for the Ohio Public Employee Retirement System.
